Healthcare’s complexity—arising from the interplay of medical practice, management, patient expectations, and chronic underfunding—demands that leaders navigate a landscape marked by both opportunity and risk.The VUCA context in healthcare is shaped by rapid demographic and technological changes, unstable funding, regulatory pressures, and unpredictable crises such as pandemics and wars. Leaders must manage under constant turbulence, balancing innovation with protection. The sector’s digital transformation brings significant opportunities: the expansion of telehealth and mobile health services, the integration of artificial intelligence and predictive analytics, and the potential for real-time clinical decision support. However, these advances are accompanied by substantial risks, including data privacy concerns, cyberattacks, ethical dilemmas, and readiness gaps among both professionals and patients.
A synthesis of 13 peer-reviewed publications from 2019–2025, identified via Scopus AI, provides the evidence base for this analysis. The research highlights that 90% of healthcare organizations still operate below level 3 digital maturity, indicating that most are reactive rather than proactive in managing digital risks. Common challenges include underfunding of cybersecurity, fragmented digital responsibilities, rapidly evolving regulations, and adaptation issues for both staff and patients. Workforce stress, lack of training, and the proliferation of fragmented digital solutions further contribute to systemic fragility.
