Description
Background. Chronic kidney disease is a growing global health concern, particularly for patients in stages IV-V. In Latvia, approximately 9000 people are affected. Pre-dialysis programmes are crucial to improving patient treatment quality. Our project is part of State Research Programme and aims to establish an effective pre-dialysis programme.Materials and methods. The project, set to begin in the October of 2024 and lasting 16 months, will include five sections with following objectives:
1. Study the roadmap and system of care for pre-dialysis patients (Figure 1).
2. Define quality indicators to implement optimal evidence-based clinical practice.
3. Develop and implement a pre-dialysis management plan.
4. Estimate the incremental cost-effectiveness ratio for the pilot project.
5. Develop: a) materials for patients; b) nurse receptions; c) a pre-dialysis education programme for patients; d) training on choice of dialysis method, early consideration of transplantation; e) organisation of a RRT access.
Results. The multimodal pre-dialysis programme will be created, introducing nurse- specialist in pre-dialysis in Latvia. Approximately 300 patients will be registered and educated during the project period. Our goals are to ensure that 80% of patients requiring dialysis have appropriate dialysis access in a timely manner and to include 20% of pre-dialysis patients in a preemptive transplantation programme. We plan to prove cost-effectiveness of pre-dialysis patient treatment using our pilot project.
Conclusion. The pilot project will improve pre-dialysis care and, once validated, could be continued and strengthened in patient care with the inclusion of a specialist nurse.
