Description
The research aims to qualitatively explore, analyse and contribute to the knowledge on the healthcare experiences of patients with at least two of the following conditions - chronic heart failure and/or chronic kidney disease and/or type 2 diabetes, identifying the main barriers, difficulties and support needed before and after diagnosis and during treatment.
