Abstract
Medial meniscus posterior root tears (MMPRTs) are significant injuries that disrupt knee biomechanics, leading to accelerated osteoarthritis if untreated. Surgical repair of MMPRTs has been shown to restore knee function and delay degenerative changes. This Technical Note details a transtibial pull-out repair technique for MMPRTs, with emphasis on the “bump sign,” an intraoperative phenomenon that indicates successful anatomic reduction. The procedure involves precise tibial tunnel placement, secure meniscal suturing using a 3-suture configuration, and transtibial fixation with a cortical button. The bump sign, observed during suture tensioning, manifests as a subtle elevation of the medial femoral condyle, confirming effective reestablishment of meniscal tension and reduction. This maneuver provides a reliable intraoperative confirmation of successful repair. The described technique is reproducible and offers a stable fixation method less prone to suture loosening.
