Abstract
Summary of Abstract: Changes in patient rights related parameters over time period of 2020–2023 are described. Decline in abuse and no improvement in emotional support was noticed.
Introduction/Background: Labour is as much physical, as emotional process. Dissatisfaction with labour can further impact postpartum period by increasing breastfeeding difficulties, postpartum depression, fear of subsequent childbirth.
Objectives: Study compares changes in childbirth practices in two periods - 2020 to 2021 and 2022 to 2023. Compared aspects were abuse during labour, respecting patients privacy, emotional support during labour, involvement in making choices.
Methods: A validated online questionnaire as part of IMAgiNE EURO study was filled out. Data were gathered using surveys covering a time period of 2020 to 2021 (Round 1) and 2022 to 2023 (Round 2). A questionnaire was filled out by patients who gave birth in healthcare facilities in Latvia, aged 18 and more. Results conclude 2079 women in Round 1 and 889 women in Round 2, accordingly. Preliminary data from time period 2022–2023 was used.
A comparative analysis was performed.
Results/Discussion: Improvement was observed in companionship during labour (63,1% in 2020–2021 to 86,1% in 2022–2023). Decline in experience abuse (verbal, physical and emotional) during labour from 17,1% to 3,3% and in making informal payments (4,5% to 1,2%). Moderate changes were observed in respecting patients' privacy (72,1% to 77,7%), treating the patient with dignity (75,2% to 78,1%) and including the patient in decision making (56,6% to 60,6%). Providing emotional support remained stable during years (68,9% to 69,6%).
Conclusion: Positive dynamics were observed in almost all parameters.
Emotional support seems aspect that needs to be improved. Even though there is not a decline during the years sufficient improvement is not observed. Emotional needs of woman should be noticed and dealt with same responsibility as with physiological changes as proposed by World Health Organization. Identification of the problematic parameters would reduce staff attitude related issues during postpartum period.
