Expression of p53 in plasma cells and its correlation with clinicalstages of multiple mieloma

Abstract

The aim of the study was to find a correlation between expression of p53 antigen and clinical stage by Salmon – Durie (S-D) and β2 – microglobulin (B2MG) level in cases of multiple myeloma (MM).
Materials and methods.
We obtained 70 patients with primary diagnosed MM from 2011 to 2014 in Riga Eastern Clinical University Hospital’s Hematology centre. All bone marrow biopsies were stained with hematoxylin-eosin, Gordon-Sweet's reticulin silver and immunostained with p53 antibodies. Statistical analyses were performed using Graph Pad Prism 5 software.
Results.
Patients mean age was (Mean ±SD) 63.61 ± 10.81 years. All patients accordingly S-D clinical staging system were divided: I A, B (20%, n =14); II A, B (43%, n =30); III A, B (73%, n =26). S-D staging ratio in patients with p53 expression was: I B (3%, n = 1): II A, B (36%, n = 12) : III A, B (61%, n = 20). But patients with lack of p53 antigen S-D staging ratio was: I A, B (35%, n = 12) : II A, B (49%, n = 18) : III A, B (16%, n = 6). A significant positive correlation by Spearman test obtained between levels of expression of p53 protein in plasma cells and S-D clinical staging system (rs = +0.5186; p < 0.0001). p53 positive group patients showed late stages by SD clinical staging system distribution than those of the p53 negative expression (p < 0.0001, Mann Whitney U test). Statistically significant positive correlation by Spearman test was found between p53 protein expression in plasma cells and B2MKB (rs = +0.449; p = 0.0005) level. Patients with p53 positive myeloma cells had statistically significant higher B2MG level 5.585 ± 4.675 mg / dl than those of the p53-negative group 3.011 ± 1.851 mg / dl (p = 0.008, unpaired t-test).
Conclusions.
Expression of p53 is a negative predictive factor in case of MM, as late stages by S-D clinical staging system are more common in p53- positive cases.
B2MG levels in serum are higher if p53 has expressed, so in p53 positive cases patient has poorer prognosis of survival.
