Fetal bowel obstruction-managing of challenging case

Abstract

Jejunoileal atresia is a rare type of obstruction of the small bowel. Diagnosis not always clear prior to birth. Prenatal ultrasound has a poor accuracy in identifying Jejunoileal atresia prenatally. Rare colonic form of atresia can cause similar symptoms associated with jejunoileal atresia. In the 2nd trimester, echogenic bowel may be the only finding, while dilated loops of bowel with peristalsis with or without polyhydramnios are findings of the 3rd trimester.
We present the case of combined bowel (small and large) atresia in 27yo woman. G1 P0 was referred for routine ultrasound at 31 weeks. During examination a slightly dilated bowel loop until 15 mm was depicted. On follow-up, progressive enlargement of the centrally located loop with peristaltic without polyhydramnios were observed. The perianal complex was clearly visualised. The boy (3 245 g) was born vaginally at 38 weeks. Laparotomy confirmed ileal (IIIb type) and colonic (5 cm from ileocecal angle) atresias. Y jejunoileal stoma and end-to-end colonic anastomosis were created. The length of small bowels left was 50 cm. At 3 months old, the stoma was closed, the measured lengths of bowels was 100 cm.
Prenatal diagnosis of bowel atresia has a positive impact on prognosis; it allows the family to accept the diagnosis, leads to prompt postnatal management. Recurrence risk is increased with jejunoileal (type IIIb) atresia.
