First tarsometatarsal joint fusion for hallux valgus: mid-term outcome

Abstract

Introduction: First tarsometatarsal joint (TMTJ) fusion for severe hallux valgus (HV), associated with first ray hypermobility or medial column instability, is commonly used surgery.

The aim of the study was to evaluate mid-term outcome of patients with severe HV deformity, treated using first TMTJ fusion.

Materials and methods: A retrospective review of patients who underwent first TMTJ fusion for severe HV, admitted in the hospital from January 2017 to December 2018, was conducted. Pre-operative and mid-term postoperative radiological examination results and patients` satisfaction was analyzed.

Results: Mid-term outcome was evaluated in 41 patient (40 female, 1 male), mean age 63 (29-77) years, 45-81 (mean 63) months after the first TMTJ fusion. Three patients had both feet HV operated using TMTJ, but 5 patients - using another method in 1-2 years.

Pre-operative mean hallux valgus angle (HVA) was 36.270, intermetatarsal angle (IMA) 13.860, distal metaarsal articular angle (DMAA) 34.140; postoperative mean HVA was 15.840, IMA 9.670, DMAA 14.180 (p>0.05).

Mean grade of the medial sesamoid position pre-operatively was 2,25, postoperatively 1,6.

Two patients had postoperative infection, one – hypertrophic scar, one – revision and one – neuritis.

Twenty five patients (61 %) were satisfied, 3 patients were partially satisfied (7 %), 6 patients were unsatisfied (15 %) with outcome. Seven patients (17 %) were lost to follow up.

Discussion and conclusions: Severe hallux valgus patients who underwent first TMTJ fusion demonstrated improvement of HVA, IMA, DMAA and medial sesamoid position. Two thirds of patients were completely satisfied with outcome. Patients are suggested to be informed about possible complications and risks after surgery.
