Abstract
INTRODUCTION: Radiofrequency catheter ablation (RFCA) is a safe and effective treatment method for arrhythmias in children. The aim of this study was to summarize and analyze the existing experience and results with the use of RFCA in the treatment of arrhythmias in children at a national level in Latvia from the introduction of the method to the present day. METHODS: Data were collected retrospectively from the electronic health records of the only pediatric tertiary hospital in Latvia. The study included all patients under the age of 18 who underwent RFCA from the introduction of this treatment method for children in Latvia in 2011 until December 2023. To collect information about long-term outcomes, patients who did not attend an in-person follow-up visit between January 1, 2024, and November 30, 2024, were interviewed by telephone. RESULTS: A total of 58 ablation procedures were performed; 64% (n=37) of patients were females. The median age was 181 (IQR 162 – 194) months, the median weight was 62 (IQR 52 – 68) kg, and the median height was 170 (IQR 160 – 176) cm. Target tachycardia was AVNRT in 25 (43.1%) patients, AVRT in 19 (32.8%), VT in 6 (10.3%) and EAT in 4 (6.9%) patients; four patients, accounting for 6.9%, did not have their diagnoses recorded. Among 56 patients with known RFCA results, overall procedural success was 85.7% (n=48). The success rate was significantly higher in patients with AVNRT (100%, n=25) and AVRT (83.3%, n=15) compared to those with VT and EAT, who had only three out of six and two out of four successful procedures, respectively (p<0.05). Median follow-up was 27.5 (IQR 15.0 – 41.0) months. Out of 48 patients with early successful ablation, tachycardia or accessory pathway recurrence was reported in 6 patients (12.5%). None of the patients developed persisting AV block requiring permanent pacing. CONCLUSIONS: In Latvia, the overall success rate of RFCA was good but lower than reported in the literature, except for AVNRT, where excellent results were achieved.
